• FormaRX does not provide medical care, diagnoses, or prescriptions
• Clinical services are provided exclusively by independent, licensed providers through partners including OpenLoop
• The provider–patient relationship exists solely between you and the provider, not FormaRX

Use of Platform

By using FormaRX, you agree that:

• You are 18+ and located in the United States
• All information submitted is accurate and complete
• Submission of an intake form does not guarantee treatment or prescription

Providers retain full discretion to approve or deny care.

Payments

• You will not be charged unless treatment is prescribed
• If prescribed, you authorize charges for medications and applicable provider services
• Subscriptions (if selected) renew automatically until canceled

Refund Policy

Due to the nature of telehealth and prescription services, refunds are limited:

• After Provider Review:
Once a licensed provider has reviewed your information, fees are non-refundable, regardless of treatment outcome.

• Prescriptions & Medications:
If a prescription is issued, all associated fees are non-refundable.
Medications cannot be returned or refunded once processed or shipped.

• Subscriptions:
Subscription fees are non-refundable once billed. You may cancel future renewals at any time.

• Exceptions:
Refunds may be granted in cases of duplicate charges or billing errors. All other requests are handled at our sole discretion.

Privacy & Data Use

We collect personal, medical, and payment information necessary to operate the platform.

Your data may be shared with:

• Licensed providers
• Pharmacies
• Clinical infrastructure partners (including OpenLoop)
• Payment and technology vendors

FormaRX does not sell your personal or health information.

We operate in alignment with applicable laws, including the Health Insurance Portability and Accountability Act where applicable. Third parties are responsible for their own compliance.

GLP-1 & Weight Loss Treatment Disclosure

If prescribed GLP-1 medications (e.g., semaglutide or similar):

• Results vary and are not guaranteed
• These medications require ongoing medical supervision
• Common side effects may include nausea, vomiting, diarrhea, constipation, and decreased appetite
• Serious risks may include pancreatitis, gallbladder disease, kidney issues, and rare but serious thyroid tumors

• Not suitable for individuals with:
o Personal or family history of medullary thyroid carcinoma
o Multiple Endocrine Neoplasia syndrome type 2 (MEN2)

You agree to provide accurate medical history and follow provider’s guidance.

In accordance with guidance from the Federal Trade Commission:

• Weight loss outcomes vary significantly based on individual factors including adherence, lifestyle, and medical history
• FormaRX does not guarantee specific results, timelines, or outcomes
• Any statements regarding weight loss, health improvements, or lifestyle benefits are for informational purposes only and should not be interpreted as guarantees

If endorsements or paid promotions are used, they will be clearly disclosed as required by FTC guidelines.

 

Third-Party Responsibility

FormaRX is not responsible for:

• Medical decisions or outcomes
• Provider actions or clinical care
• Pharmacy fulfillment, delays, or errors
• Third-party systems, data handling, or availability

 

Limitation of Liability

To the fullest extent permitted by law, FormaRX disclaims liability for:

• Medical outcomes or treatment decisions
• Data breaches outside reasonable control
• Service interruptions or technical issues

All services are provided “as is” and “as available.”
